Martin Avenue Pharmacy requests that all custom compounded medications
be called in 2 to 3 days in advance of when you intend to pick up
your custom made medication. This allows us time to compound your
medication and check with your physician if needed.
In order to expedite the processing of your prescription order, please have the following information available: patient first and last name, prescription number, and telephone number where you can be reached.
Your physician may require you to schedule an appointment with him / her before your custom compounded medication may be refilled. An office visit is very important and ordered for your safety. If an office visit is necessary, Martin Avenue Pharmacy will notify you.
